The skin concerns hiding beneath the surface

The case for objective skin assessment and why treating what is visible is only part of the picture

Most skin concerns are identified visually. A client notices a change, sees something in the mirror that was not there before, or becomes aware of a concern that has been developing gradually over time. That observation is a starting point, but it doesn’t tell the full story. 

What is visible on the skin's surface represents only a fraction of what is happening within it. Think: pigmentation that has not reached the surface, vascular activity that influences redness and reactivity and deeper levels of skin laxity, hydration, and texture.

At TAE, clinical imaging is used across both skin and aesthetic medicine consultations. For clients attending for cosmetic treatments, standardised imaging forms part of every appointment, creating a consistent visual record of facial structure, volume and skin quality over time. 

This is not standard practice in most aesthetic clinics, and it is one of the features that shapes how treatment planning and result tracking works at TAE.

What the Clinical Imaging System captures

TAE uses the Clinical Imaging System, a standardised skin analysis platform used across dermatology, plastic surgery and non-surgical aesthetics clinics internationally.

The system uses dedicated lighting that eliminates variables like ambient light and time of day, producing consistent, comparable images across every visit. This consistency is what makes objective tracking possible. When images are taken under the same conditions at every appointment, changes in the skin can be measured accurately.

Where it moves beyond standard photography is in its dermatoscopy filters. These isolate melanin and haemoglobin in the first two millimetres of the skin, creating different views from a single image. Subsurface pigmentation that has not yet appeared can be identified and assessed early. Vascular activity driving redness can be distinguished from pigmentation. This matters because the two require entirely different treatment approaches.

Why the distinction between melanin and haemoglobin matters

Pigmentation and redness can look similar on the surface. Both present as discolouration. Clinically, they are driven by different processes.

Melanin concerns involve overproduction of pigment by melanocytes, triggered by UV exposure, hormones or post-inflammatory change. Haemoglobin concerns involve vascular activity, the driver behind redness, broken capillaries and conditions like rosacea.

Applying a treatment designed for pigmentation to a vascular concern produces little result, and vice versa. The imaging filter that separates these two signals removes the guesswork entirely. What appears as general redness on the surface can be assessed for its specific vascular versus pigmentary components, informing which modality is appropriate and in what order.

Tracking results objectively over time

Skin improvement is gradual. Results from a course of clinical treatment develop over weeks and months, and subjective assessment of whether the skin has improved is unreliable.

Standardised imaging throughout your treatment plan creates a visual record. Before and after comparisons can be made under identical conditions, showing changes in pigmentation, vascular activity, texture and overall skin quality in a way that is clearly measurable.

For clients undergoing treatment for pigmentation or vascular concerns, this is particularly valuable. It allows the team to assess whether the treatment is producing the expected response, adjust where needed and show progress that the client may not yet see clearly day to day.

What this means for how skin is assessed at TAE

Every new client at TAE receives a skin assessment using the Clinical Imaging System as part of their initial consultation. The images captured form the baseline for all subsequent treatment planning and tracking.

For clients attending for aesthetic medicine appointments, the system is used to document facial structure and changes in volume, definition and skin quality at each visit. Comparing images taken under identical conditions at every appointment allows for a level of precision in treatment planning and outcome tracking that visual assessment alone cannot provide. It also means the client has a clear, objective record of their results over time.

For clients with pigmentation concerns, the subsurface view often reveals damage that has not yet surfaced, determining both the urgency and sequencing of treatment. For clients with vascular concerns or persistent redness, the haemoglobin filter clarifies whether the presentation is primarily vascular, inflammatory or pigmentary, directly influencing which treatments are appropriate.
